We are honored to make an investment in your organization because your initiative is aligned with our goal of making North Carolina a healthier place to live. The BCBSNCF grant process is underway and there are several steps to follow in order to become a grantee: 1) Review the information in this letter, 2) Sign the Equipment Grant Agreement (information below), 3) Participate in a New Grantee Webinar (information below), and 4) Provide a Project Summary for your project (250 words or less) and the impact it will have on healthy eating in your community, and 5) Work with NCRPA to refine your results statement. The Equipment Grant Agreement will be sent in a separate email. Please review it carefully, as it outlines the conditions, policies, and terms of the grant. The grant agreement will have to be signed by your Project Director (in lieu of an Executive Director) and Director of Parks and Recreation or Director of Finance (in lieu of the the Board Chair). Please scan the signed grant agreement and submit it online by logging in to the online grant portal (https://www.grantrequest.com/SID_1303?SA=AM) and completing the Grant Agreement Submission Requirement. The New Grantee Webinar is scheduled for April 30 at Noon. You will be able to access the webinar at the following link: https://bcbsncfoundation.globalmeet.com/hac. If you cannot attend the webinar, we will post a recording online (http://bit.ly/17bzt62) under the Key Dates section which you will need to access by May 2. The New Grantee Webinar will welcome you as an official grantee, offer some recommendations for refining your results statement, assist you with creating your project summary and prepare you for a grant announcement in your local media. Please do not announce the grant externally until you have participated in the webinar. The Project Summary will be completed by logging in to the online grant portal (https://www.grantrequest.com/SID_1303?SA=AM) and completing the Grantee Onboarding Requirement. All equipment grantees will submit a final report at the end of the grant. You will receive a request for your final report nine months from now, with the report due one year from now. If you are done early and have spent the grant dollars given to you, then you are welcome to submit the report before that date. The report should take about 30 minutes to complete and will have to be signed by your Project Director (in lieu of an Executive Director) and Director of Parks and Recreation or Director of Finance (in lieu of the Board Chair). We also ask that you fill out an End of Grant narrative form that will summarize the experience and impact on your community. You may use this summary as communication material for your organization.
